Calibrate your machine and set it up properly.
First the mechanical basics need to be top before messing with slicing settings and other software things.

Looks like a severe case of backlash, infill not touching the perimeter on one or two sides, infill lines are in pairs with a gap between pairs. Hold the x or y motor and move with your other hand the hot end or printbed. Is there a lot of play, or springiness? Are your belts tight? Is the print surface firmly clamped down? Is the hot end firmly attached to the x carriage or extruder? Are there idlers moving up/down when the direction is changed? Are the linear bearings tight without play?

What about the acceleration? First layer print speed? Try 1500mm/s2 as acceleration for x, y and e. z depends on type of printer. First layer speed betwern 15 and 20 mm/sec.

Once you've fixed this issue, you can try adjusting the z offset for the proper first layer height, then adjust the extrusion multiplier.
