Locked nut in wade's extruder giving problems

In my current setup I've an wade's geared extruder, with an hobbed bolt like this; [www.flickr.com]

I used two of those locked nut's (like in the background of the image), but for some reason the big gear comes loose under stress. Any thoughts on how to fix this? This would be the last hurdle for me to take.

Use two nuts if there is room, thus locking both nuts together.

There was no spare room for a second (or actually this hobbed-bolt design would use 4 nuts). I made an new one from a regular bolt like in the original documents. This one works perfectly.

One last alternative, though a pain, is to grind down two nuts so you have two thin nuts.

fyi, locknuts are single use only. We all use them over and over but they are not designed for that. Sublime has an excellent idea, locktite.
