Here's a trick that I've been using to get reproducible extruder start at the beginning of each print. Home the extruder before turning on the extruder heater. With the nozzle near the bed (0.1mm away in my case), it minimizes ooze. Combined with skirt and retract/restart in Skeinforge, this results in consistent extrusion at the start of the print without having to prime the nozzle by extruding in air in between prints. Note that SF is set to retract at the end of each print by default.
